Steps to Completing an Evaluation

Start by filling out a New Evaluation Consult Form. You will be able to include why you are seeking the evaluation, as well as insurance/payment information. You will be placed on the waitlist after completing this form.

Waitlist and Intake Scheduling

Our current waitlist is about 3-5 months for adults and 12+ months for children from the time of completing the form to the Intake Appointment with the Psychologist. Our Evaluation Coordinator will call you to schedule your Intake appointment approximately 1 month in advance.

Attend Intake and Testing Appointments

After you attend your Intake Appointment with the Psychologist, they will decide what testing is needed for your evaluation. You will schedule & attend additional appointment(s) to complete the performance-based testing & questionnaires.

Receive Report & Schedule Feedback

After all testing is completed, your Psychologist will write a comprehensive report- they will let you know when to expect this. Once you receive your report, you can schedule a Feedback session with the Psychologist to go over the results & recommendations.
